Camp Forms

Licensed by various state agencies, Sonlight is required to have specific forms on file for each camper and staff member. This information allows our Health Care Team and staff to provide the best care we can for your camper. We have worked to streamline the process to be as easy as possible for parents, we do know that it’s a lot!

All required forms are in you Parent Dashboard.  Using these downloads eases the processing in the office as the downloads are marked with barcodes that then upload directly to your child’s record.

Forms Required for all Campers

• • Online Camp Registration form with complete contact information for parents / guardians and alternate contacts- If you’re registered, we have this!
  • Online or Paper Health History Every camper’s parents need to complete and submit a current health form each year. Through the Parent Dashboard, you may complete this form online or download the form and return via email or fax.
  • Paper Physical Form signed by a physician, nurse practitioner or physician assistant.

    The regulations by which Sonlight has to abide by have changed allowing us to accept Physical Exam Forms dated within 24 months of start of your child’s camp.  Our staff will be going through last year’s records and marking the Physical Exam Form received if we have a valid Physical on file for your child.  It will then show as “received” on your Parent Dashboard.  We hope to have these all entered by April 15^th.

     If your child’s health history has notably changed or their necessary medications have changed, we will need you and your child’s physician to complete a Medication Permission Form to be added to your child’s Physical for this year.  All medications (prescription, over the counter, or homeopathic) that you would like your child to receive while at camp MUST be listed on their Physical Exam form or on a Medication Permission Form AND arrive at camp in its original packaging.  

    State child care regulations require the Physical Form to be signed by a Medical Doctor, Nurse Practitioner or a Physician’s Assistant. Other practitioners such as chiropractors or naturopaths are not accepted at this time. This form may be downloaded through your Camp In Touch dashboard.

  • Paper Immunization Form. Our licensing regulations through the State of Colorado require that all immunizations records be submitted on the Colorado Certificate of Immunization, regardless of the camper’s state of residence. This form is also available in your Parent Dashboard under Forms and Documents. Once we have these in our files, they may be carried over year to year, unless there is a change. Please see the Required Immunization document in your Parent Dashboard for more information.

Medication Forms – either prescription or over the counter

• • Medications MUST be listed on the Physical Form signed by the MD, PA or NP, or have the Permission for Medication Administration form completed.

Asthma Inhalers

• • An Asthma Self-Carry Contract  signed by parents and the camper is required for any camper who will bring an inhaler to camp.

Epi-Pens (or epi-autoinject)

• • A Colorado Allergy and Anaphylaxis Emergency Care Plan and Medication Orders AND Allergy Self-Carry Contract are required for any camper coming to camp with an auto-inject epinephrine device (Epi-pen)

Authorization Forms

• • This form provides permission for Sonlight to seek medical treatment, or transport your child.  Additionally, is includes required, non-guardian, emergency contacts and permission for special activities.
